Create a Quiz - Microsoft Forms

Overview

Quizzes are a great way to assess knowledge, evaluate learning, or create interactive experiences. Microsoft Forms makes it easy to create quizzes with automatic grading, instant feedback, and customizable questions. Follow this step-by-step guide to create your first quiz.

Start a New Quiz

  1. On the Microsoft Forms Dashboard, click New Quiz.

    1. By selecting a template, you can choose the Quiz option to generate a pre-designed quiz with questions tailored to your topic.

  1. Give your quiz a title and description (e.g., "Math Quiz" or "Employee Training Assessment").

    1. Select the image icon to the right to add a company logo.

Add Questions

Like creating a form, quizzes also offers a variety of question types to suit your quiz needs. To add questions:

  1. Click Quick start with to insert a question.

  2. Choose a question type:

    • Choice: Multiple-choice or checkbox questions (e.g., "What is 2 + 2?")

    • Text: Open-ended text responses (e.g., "Explain the concept of gravity.").

    • Rating: Star or number-based rating scales (e.g., "Rate your confidence in this topic.").

    • Date: Collect date-based responses (e.g., "When did World War II end?").

    • Ranking: Allow respondents to rank items in order of preference.

    • Likert: Use a scale to measure attitudes or opinions (e.g., "Strongly Agree to Strongly Disagree").

    • Upload File: Allow respondents to upload files (e.g., "Upload your project submission as a PDF").

    • Net Promoter Score (NPS): Measure loyalty or satisfaction with a 0–10 scale (e.g., "How likely are you to recommend this course to a friend?").

    • Section: Organize your quiz into multiple parts with headings and descriptions (e.g., "Section 1: Basic Math Questions").


Customize Questions

Customizing questions are also similar to creating a form, however, quizzes offer a few more details and options to best tailor the quiz for your students needs.

  1. Add images or videos to make questions more engaging.

  1. In the toggle menu located at the bottom, there are a few differences compared to creating a form.

    1. Mark questions as Required if respondents must answer them, or provide the option for Multiple Answers.

    2. Select Math to display math symbols and formula options, then select Enter an equation.

  1. Select the More Options (•••) icon to the furthest right for more customizable options such as making the options a Drop-down, or to provide Branching Logic to create personalized paths based on response.


Assign Points and Correct Answers

For each question, you can assign points and specify correct answers:

  1. Click the Points field to assign a point value to the question.

  1. Select the Correct Answer check mark next to the correct answer or answers

  2. Select the Message icon next to any answer if you want to customize a message for it. Respondents will see the message when they've selected that answer.

  1. For text questions, specify the correct answer(s) in the Correct Answer field.


Preview Your Quiz

Before sharing your quiz, preview it to ensure it looks and functions as expected:

  1. Click the Preview button to see how your quiz will appear to respondents.

  1. Test the quiz by answering the questions yourself.

  2. Make adjustments as needed.


Adjust Settings

Customize your quiz settings to control how it’s shared and how responses are collected:

  1. Click Settings.

  2. Configure options for quiz:

    • Practice Mode: Respondents can practice at their own pace and see correct answers for each question before submitting.

    • Show results automatically: Responders will see their results and correct answers immediately after submitting the quiz.

  3. Configure other options such as:

    • Recipients: Choose who can fill out this quiz.

    • Start and End Dates: Set a time frame for when the quiz is available.

    • Response Limits: Limit the number of responses.

    • Anonymous Responses: Allow respondents to submit anonymously.

    • Response Receipts: Configure receipts and email notifications upon response submissions.


Share Your Quiz

Once your quiz is ready, share it with your audience:

  1. Click Collect responses to generate a link, QR code, or embed code.

  1. Specify the recipients who respond to the quiz.

    • Anyone can respond (unlike creating a form, this option is not available for quizzes)

    • Only people in Seattle Pacific University can respond

    • Specific people in Seattle Pacific University can respond

  2. Distribute the quiz via:

    • Copy a link

    • Outlook and Microsoft Teams (as a chat)

    • Download QR code

    • A website or blog (using the embed code)

Collect and Analyze Responses

After sharing your quiz, monitor and analyze the responses:

  1. Go to the Responses tab to view real-time results.

  1. Export responses to Excel for further analysis by clicking Open in Excel.

  2. Select Review answers to review individual responses.

  3. Select Post scores to give respondents access to view feedback and the results of their quiz.

    • Students can view their scores and feedback by using the same link that they used to launch the quiz.

  4. Use the automatically generated charts and summaries to gain insights.

    • Select more details to gather specific insights for each question

  5. You can share response summaries with others:

    1. Click the Option menu (…) in the top-right corner.

    2. Select Share a summary link to generate a link to the summary, which includes charts and key insights.
