  • Issue: The form doesn’t load, or respondents can’t access it.

  • Solution:

    • Check your internet connection.

    • Ensure the form link is correct and hasn’t been altered.

    • Verify that the form is shared with the correct audience (e.g., public, specific people, or within SPU only).

2. Responses Not Being Collected

  • Issue: Responses aren’t appearing in the Responses tab.

  • Solution:

    • Ensure the form is still open and hasn’t reached its response limit.

    • Check if the form settings allow anonymous responses or restrict submissions to one per person.

    • Verify that the form hasn’t been accidentally closed or deleted.

3. Branching Logic Not Working

  • Issue: Conditional questions (branching logic) aren’t functioning as expected.

  • Solution:

    • Double-check the branching logic setup to ensure the correct paths are defined.

    • Test the form thoroughly in preview mode before sharing it.

3. Can I edit a form after it’s been shared?

  • Yes, you can edit a form after sharing it. However, changes may affect responses already collected, so proceed with caution.

4. How do I allow anonymous responses?

  • Go to the form’s Settings and ensure Record name is not enable . Note that this prevents you from identifying individual respondents.

5. Can I use Microsoft Forms offline?

  • No, Microsoft Forms requires an internet connection to create, share, and respond to forms.

6. How do I delete a form or responses?

  • To delete a form, go to My Forms, select the form, and click Delete.

  • To delete responses, go to the Responses tab and click Delete All Responses or remove individual responses.
