Required Information for a W-9/W-8BEN Request Form:

  • Contractor/Vendor Name

  • Contractor/Vendor Email

  • Is the vendor located within the United States? (Vendors located outside of the United States will receive a W-8BEN form. The rest of the process is the same as vendors within the United States.)

  • Grant Name

  • Fund Name (Usually begins with 115 or 243.)

  • Principal Investigator Name

Once you have gathered the above information, use the W-9/W-8BEN request form to submit this information to OSP. A secure request form will be sent to the vendor.
