Providing Documentary Proof of COVID-19 Vaccination

SPU employees should take the following steps to provide documentary proof that they are fully vaccinated against COVID-19:
     Log into the secure Patient Portal with your SPU credentials.
     Select the Immunization tab in the top row.
     Click on Required Immunizations. If COVID-19 vaccination dates do not already appear, then manually enter the dates of the applicable COVID-19 vaccination that you received, then click Submit.
     Next, click the Upload tab. Select Immunization History from the dropdown menu, then upload a copy of your supporting documentation.

After you have provided your vaccination dates and uploaded your supporting documentation, Health Services staff will review the information you have submitted.

Requesting a Medical or Religious Exemption

An SPU employee who wishes to apply for a religious or medical exemption from SPU’s COVID-19 vaccination requirement may take the following steps:
     Download the Medical Exemption Request Form or the Employee Religious Exemption Request Form (as applicable), then complete the form.
     After you have completed the form, log into the secure Patient Portal with your SPU credentials.
     Next, click the Upload tab. Select either Waiver Medical Immunizations or Waiver Religious Immunizations, as applicable, then upload a copy of your completed exemption request form.

After you have provided your vaccination dates and uploaded your supporting documentation, designated SPU administrators will review the information you have submitted and respond to you whether your exemption request has been granted.

Disclosure of Information

SPU treats vaccination and exemption information submitted to Health Services as confidential. However, in order to enforce legal requirements and institutional policies related to COVID-19, Health Services will need to disclose to other SPU officials whether employees have been fully vaccinated or have been approved for an exemption. For example, supervisors may be notified of unvaccinated employees in their areas who should wear face coverings. By submitting vaccination information to SPU, and by submitting an application for an exemption, each SPU employee consents to Health Services disclosing to other SPU officials as needed to fulfill legal requirements and institutional policies whether such employee is fully vaccinated or has been approved for an exemption.
