All SPU employees are required to be fully vaccinated against COVID-19 and to submit documentary proof of vaccination by August 16, 2021, unless an exception is approved. 

Providing Documentary Proof of COVID-19 Vaccination

An SPU employee who wishes to apply for a religious or medical exemption from SPU’s COVID-19 vaccination requirement may take the following steps:
     Download the Medical Exemption Request Form or the Employee Religious Exemption Request Form (as applicable), then complete the form.
     After you have completed the form, log into the secure Patient Portal with your SPU credentials.
     Next, click the Upload tab. Select either Waiver Medical Immunizations or Waiver Religious Immunizations, as applicable, then upload a copy of your completed exemption request form.

After you have provided your vaccination dates and uploaded your supporting documentation, designated SPU administrators will review the information you have submitted and respond to you whether your exemption request has been granted.The Office of Human Resources will coordinate review of exemption requests in consultation with other SPU administrators and will notify you if your exemption is approved. 

Disclosure of Information

SPU treats vaccination and exemption information submitted to Health Services as confidential. However, in order to enforce legal requirements and institutional policies related to COVID-19, Health Services will need to disclose to other SPU officials whether employees have been fully vaccinated or have been approved for an exemption. For example, supervisors may be notified of unvaccinated employees in their areas who should wear face coverings. By submitting vaccination information to SPU, and by submitting an application for an exemption, each SPU employee consents to Health Services disclosing to other SPU officials as needed to fulfill legal requirements and institutional policies whether such employee is fully vaccinated or has been approved for an exemptionUnder guidance from Governor Inslee for institutions of higher education and from the Department of Labor and Industries for employers, SPU is required to verify the vaccination status of students and employees and enforce face covering requirements. While SPU generally treats employment records and education records as confidential, SPU will share vaccination and exemption information it receives from students and employees as needed with SPU officials in order to fulfill legal requirements.
